OCD isn't only about visible rituals or tidiness, and it isn't a personality quirk. It often centres on intrusive, unwanted thoughts that cause real distress, along with compulsions (visible or entirely internal) aimed at neutralising that distress.
A lot of the work is simply making room to talk about the actual content of the thoughts without judgement — often the first time someone has been able to say it out loud — and understanding the cycle that keeps it going.
